Q: How do I apply for examination qualification certificate re-issuance?

    For urgent certificate re-issuance, a qualified applicant or trusted person (accompanied by a Letter of Attorney) may come in person for certificate re-issuance in under an hour. Certificate re-issuance request via mail will take 3 working days to process from the date of receipt and will be mailed to the applicant via registered mail. Receiver:Section 1 Division 2 of Examination Yuan. Address:No.1, Shiyuan Road, Wenshan District, Taipei City. Contact number: (02)82366209.

    Mandatory documents required for certificate re-issuance:
(1)One copy of application for re-issuance of misplaced or damaged Examination Yuan examination or training qualification certificate (PDF format), (Word format). Applicants may refer to Figure 1for reference. Applicants may also request a copy of the application by sending a self-addressed stamped envelope, fax or download one from the internet and print on A4 sized paper.
(2)Damaged certificate re-issuance applicants please attach the original copy of the certificate.
(3)One copy of R.O.C. ID both front and back.
(4)$500 NTD processing fee (please pay via money order to: Secretariat Cashier Section of Examination Yuan. )
※Remarks:
◎R.O.C. citizens living overseas or foreigners should specify nationality and passport number under the ID column.
◎After the certificate has been issued, the original shall be void. Any recovered original certificate must be returned to the Examination Yuan for registration cancellation.
